But it is the beginning — not the end.
The period following residential treatment is both the most important and the most vulnerable phase of recovery. The structure and safety of the residential environment are gone. Normal life — with its stressors, relationships, triggers, and demands — is back. The skills built in treatment now need to be applied in the real world, often under pressure.

What Is Aftercare in Addiction Treatment?
Aftercare is structured professional support provided after residential treatment to help people maintain and strengthen their recovery over time.
It is not a soft add-on. It is a clinical necessity.
Research consistently shows that longer engagement with the treatment system after residential care significantly improves long-term outcomes. The aftercare period is when the real work of integrating recovery into life actually happens.
Why the First 90 Days After Rehab Are Critical
Within the first 90 days of leaving residential treatment, relapse risk is at its highest. Several factors explain this:
Environmental Return
The environments, people, and social contexts associated with past use are present again. Environmental cues are powerful neurological triggers — even after weeks of abstinence, a particular street, bar, or face can produce intense craving.
Loss of Structure
Residential treatment is highly structured. That structure is protective. Without it, the person must build their own structure — which takes time and active effort.
PAWS
Post-Acute Withdrawal Syndrome produces ongoing low mood, sleep disturbance, difficulty concentrating, and persistent cravings for weeks to months after acute withdrawal ends. These symptoms create ongoing vulnerability.
Relationship and Practical Stress
Returning to relationships damaged by addiction, navigating employment challenges, managing finances — these are real stressors that create pressure during an already vulnerable period.

Regular Individual Sessions
Regular individual sessions with your aftercare counsellor provide: – A structured space to review how recovery is progressing – Accountability and consistent support – Early identification of warning signs – Relapse prevention plan review and adjustment
These sessions continue throughout the 12-month period.
Group Aftercare Sessions
Group sessions bring together people who completed residential treatment — both recently and further back in their recovery. This community is valuable: – Normalising the challenges of post-residential life – Peer support from people who genuinely understand – Shared strategies for managing specific situations – A sense of ongoing connection
Crisis Access
During aftercare, clients have access to their counsellor outside of scheduled sessions when a crisis arises. Knowing that support is available — not just on Thursdays at 3pm — is itself protective.
Family Support Continuation
Family therapy may continue into the aftercare period where indicated. Relationships rebuild over time, and ongoing family support helps this process.
Relapse Prevention Plan Reviews
The relapse prevention plan developed during residential treatment is reviewed regularly. Life circumstances change. New stressors arise. The plan must adapt.
Support Group Connections
Aftercare supports engagement with external peer support networks (AA, NA, SMART Recovery, faith-based groups) as a supplement to professional aftercare.
What Aftercare Looks Like Month by Month
Months 1–3
Highest intensity of support. More frequent sessions. Focus on managing the immediate transition — new environment, relationships, work, stress. High vigilance for early warning signs.
Months 4–6
Support continues as the new life structure becomes more established. Work on PAWS symptoms as they arise. Relapse prevention plan refined based on real-world experience.
Months 7–12
Gradual consolidation. The person has built genuine recovery infrastructure. Aftercare sessions may become less frequent but remain consistent. Focus on long-term life planning and continued growth.
What If I Relapse During Aftercare?
A relapse during aftercare does not end the aftercare programme. Our response is clinical and compassionate: understand what happened, learn from it, strengthen the plan.
For some relapses, a return to residential treatment is the appropriate clinical response. For others, intensification of aftercare support is sufficient. Our aftercare team makes this assessment collaboratively with the client.
Remote Aftercare for International Clients
For clients who return to their home country after residential treatment in South Africa, our aftercare programme includes remote options — video sessions with your counsellor. Recovery support continues regardless of geography.

Is aftercare free at HHG? Yes. All HHG facilities include 12 months of aftercare support as part of the residential treatment package.
How often are aftercare sessions? This is individualised based on clinical need — more frequent in early months, typically tapering as recovery strengthens.
Can I start aftercare if I didn’t complete residential treatment at HHG? Contact us to discuss. In some cases we can provide aftercare support to people who completed treatment elsewhere.
What if I move after completing residential treatment? Remote aftercare is available for clients who relocate. Geographic distance does not need to end aftercare support.
Is 12 months of aftercare enough? For most people, 12 months of active aftercare provides an excellent foundation. Some people benefit from ongoing support beyond this period, which can be arranged.
